Notice of Infringement – Claim
To file a DMCA takedown notice, you must include the following details:

- Identification of the infringing material, including the URL of the page where the material is located, to help us identify and remove the content.

- A statement under penalty of perjury that the information in your notice is accurate and that you are authorized to act on behalf of the copyright owner.
Note: Under Title 17 USC §512(f), any person who knowingly and materially misrepresents information in a DMCA notice may be subject to civil damages, including costs and attorney fees.

Counter Notification – Restoration of Material
If your content has been removed due to a DMCA takedown notice and you believe it was a mistake or misidentification, you may submit a counter-notification to have the material restored. Your counter-notification must include the following elements, as required by 17 USC Section 512(g)(3):
- Your physical or electronic signature.
- A description of the material that was removed and its original location on the Site before removal.
- A statement under penalty of perjury that you have a good faith belief the material was removed or disabled due to a mistake or misidentification.
- Your contact information, including your name, address, and telephone number, along with a statement consenting to the jurisdiction of the federal district court where your address is located (or any judicial district in which the service provider may be found, if outside the United States). You must also agree to accept service of process from the party who submitted the original DMCA notice.

Repeat Infringer Policy
